If you are a graduate student who received an admission at UCI, the following completed documents must be submitted to your respective departments in order to obtain an I-20 document for an F-1 student visa.

1. Admission letter (usually the departments has this letter)
2. Confidential International Applicant Questionnaire (CIAQ) form: This form should have been mailed to you by your respective departments or school. If not, you may download one from the International Center’s web site: http://www.ic.uci.edu/forms/CIAQ2006.pdf
3. Financial Statement: A financial statement may consist of your personal bank statement, your parents’ bank statement, another sponsor’s bank statement or an award letter provided by your respective departments or school. Your financial statement must be valid for at least 6 months from the day the International Center receives it. A financial statement may be an official personal bank statement, money market account statement, mutual fund and/or retirement plan statement. If someone is providing you financial support, you must provide us with an official bank statement and affidavit letter of support. You may also have your bank or your sponsor’s bank certify the 4th page of the CIAQ form. Please be advised that you will need to obtain an official statement when you are applying for an F-1 visa at your home country’s embassy/consulate.

The above documents must be completed and must be submitted to your respective departments/school. Once your respective departments receives your complete documents, a request form and your documents will be forwarded to the International Center for I-20 processing. Please be advised that there is a processing time of 15 business days.

Please be advised that if you are a transfer student from another U.S. institution, you must follow the transfer procedure prior to obtaining an I-20 document.
