CURRENT STUDENTS

If requesting I-20 Extension, BOTH forms need to be completed and submitted to the IC:

• I-20 Request Form (to be completed by STUDENTS)
• Academic Advisor's Recommendation for Extension and Change of Major (to be completed by ACADEMIC ADVISOR/COUNSELOR)
• Part Time Petition Form (Students Only)
• Form B: Request for DS2019:  (to be completed by J-1 Graduate Students)
• Student / Scholar Departure Form

NEW SCHOLARS
If requesting DS-2019, BOTH forms need to be completed and submitted to the IC:

• Form A: DS2019 Request Form (to be completed by SCHOLARS)
• Form B: Department’s Sponsor (to be completed by DEPARTMENT)
